APX Create Default Codeplug Operation

The Create Default Codeplug operation will create a default codeplug for a radio of a given model, firmware version, and codeplug version.
A .duf can optionally be used to supply parameter values from an existing radio. The No File option is also available.

IMPORTANT! - New Serial Number is only available for radios with firmware lower than R35

The number of required parameters is different for the selected option ("No File" or "Choose Duf File"/"Read Radio").
